Dear Patients,
It is with mixed emotions that I announce my retirement from active practice effective 12.31.2025. I will no longer be able to provide medical care after that date. It has been a privilege to serve as your physician all these years and it is not easy for me to give it up.
If you have not already received a referral to a new clinician, please contact the office at 850.877.0635, option 3 to ask for one. Alternatively, a referral may be received at your next scheduled appointment. I recommend you contact them as soon as possible to schedule an appointment so there is no lapse in your care.
Your medical records are confidential and will remain on file at my practice location of 1407 M D Lane, STE A, Tallahassee, FL 32308 for the next 5 years after the date of your last appointment OR until you request their transfer to another clinician or yourself. We will assist you in the transfer of records directly to your new physician, there will be some authorization paperwork involved. You will be asked to complete a Release of Information form when you are ready for records to be transferred. This release must also be filled out if you are requesting a copy of your records for your own self; please keep in mind there will be a processing charge for records delivered to a patient or their guardian.
