How to Become a New Patient

All new patients must complete a new patient intake form, which will be reviewed by the corresponding provider. We’ll determine whether an appointment at our office will be beneficial for you. We strive to ensure that each patient receives the most appropriate care tailored to their unique needs.

1. Complete New Patient Request

You have two convenient options:

Online: Fill out and securely submit our online patient request directly to our office. Our admissions team will review your information and contact you directly to discuss next steps and schedule your appointment.

By Phone: Call us at (850) 205 0189 to provide your information and schedule your appointment.

2. Before Your First Appointment

Once your appointment is scheduled by our admissions team, you will receive new patient forms via the patient portal. Please complete these forms at least 48 hours prior to your appointment. We’ll contact you two business days beforehand to confirm your appointment.

3. First Appointment

For scheduled appointments, please arrive 30 minutes early to complete paperwork (unless it has already been completed at home). We’ll contact you two business days beforehand to confirm your appointment.
